Job Responsibilities
As a Nurse Family Partnership Nurse, your primary responsibilities will include:
- Conducting home visits to assess the health and well-being of mothers and their newborns.
- Providing education on prenatal and postnatal care, infant care, and parenting skills.
- Creating individualized health plans in collaboration with each family to address their specific needs.
- Monitoring the growth and development of children and ensuring they receive necessary medical care.
- Building strong relationships with families to foster trust and open communication.
- Collaborating with community resources and services to connect families to additional support.
- Documenting all interactions and maintaining confidential records in compliance with regulations.
- Attending regular team meetings and training sessions to enhance skills and knowledge.
Qualifications
To be considered for this position, candidates must meet the following qualifications:
- Current nursing license in the state, with a minimum of 2 years of nursing experience.
- Experience in maternal and child health, home health, or community nursing preferred.
- Strong verbal and written communication skills, with the ability to connect with families from diverse backgrounds.
- Ability to work independently and manage time effectively.
- A valid driver’s license and reliable transportation for home visits.
- Certification in CPR and First Aid.
- Basic proficiency in electronic health records and computer systems.
