Job Responsibilities

As a Nurse Family Partnership Nurse, your primary responsibilities will include:

Conducting home visits to assess the health and well-being of mothers and their newborns.
Providing education on prenatal and postnatal care, infant care, and parenting skills.
Creating individualized health plans in collaboration with each family to address their specific needs.
Monitoring the growth and development of children and ensuring they receive necessary medical care.
Building strong relationships with families to foster trust and open communication.
Collaborating with community resources and services to connect families to additional support.
Documenting all interactions and maintaining confidential records in compliance with regulations.
Attending regular team meetings and training sessions to enhance skills and knowledge.

Qualifications

To be considered for this position, candidates must meet the following qualifications:

Current nursing license in the state, with a minimum of 2 years of nursing experience.
Experience in maternal and child health, home health, or community nursing preferred.
Strong verbal and written communication skills, with the ability to connect with families from diverse backgrounds.
Ability to work independently and manage time effectively.
A valid driver’s license and reliable transportation for home visits.
Certification in CPR and First Aid.
Basic proficiency in electronic health records and computer systems.
